Application for Corn Inspection

 

The Department of Agriculture is designated as the official certifying agencies for certifying seeds for the State of Hawaii (HRS 150-2). Seeds are certified to maintain and make available sources of high qualify seeds and propagating material of plant varieties so grown and distributed as to insure genetic identity and genetic purity.

To meet the needs of the seed corn industry, Applications for Corn Inspections are now available in two different formats.

  1. Form to be mailed or faxed - Application for Corn Inspection in PDF format is available. Click here to download form. Send to:

  1. Forms submitted through e-mail. A Microsoft Excel worksheet is available for you to enter your information and submit it through e-mail. To do so, please complete the following steps:
  1. Download Authorized Officials for Corn Inspection form (pdf format). Complete form and mail to:

  1. Once received and approved, you will receive your password and instructions through the mail. The Excel file will be e-mailed to the address you provided on the Authorized officials for Corn Inspection form. Samples of the Excel worksheets in pdf format are available for your viewing: Application for Corn Inspection, Application for Corn Inspection Information and Confidential Plant Description.
  1. To request crosses and/or inbreds be withdrawn from the seed certification program, download Notification of Seed Certification Withdrawal Form and mail to the address provided on the form.
